Job Title: Patient Care Coordinator-Phone, PCC-
Starting Rate $18.00-$19.50
Patient Care Coordinator job description: As a Patient Care Coordinator it is your responsibility to serve our patients on phones, by strategically scheduling patients for appointments and assisting them with their requests. We will continuously strive to communicate with our CCAO team ensuring that we are taking care of patient’s needs.
Responsibilities to Include But Not Limited To:
· Have a positive and kind attitude with each other and with our patients.
· Operates multiline telephone system. Answers incoming calls in a warm and caring manner, determines purpose of callers, and forwards or flags appropriate department.
· Verify the schedule each morning looking for any issues, verifying insurances, Medicaid, referrals and PA’s for all appointments on the schedule.
· Maintain a clean, neat, and tidy office by utilizing opening and closing checklist.
· Greets and directs patients, salespeople, and visitors in a friendly manner
· Check in patients on time. If a patient is late for appointment, coordinate with the provider to get patient checked-in or rescheduled.
Handles patient registration, verifies patient’s records are up‑to‑date and accurate.
· Scan insurance cards and photo ID’s into system and attach to patient registration.
Receives payments and past due balances. Enters into system. Tallies daily deposits for the day. Balance petty cash in the morning and evening. This includes over the phone payments
· Check-out patients ensuring follow up appointments are schedule. If patient does not schedule future appointment set up recall and future flag yourself for follow up.
· Schedule patients for all appointments (New Patient, Return, Infusions, phlebotomies, and Injections etc.)
· Send proper flags to each depart (PA, Referrals, infusions/injections, billing)
· Confirms new patient appointments for next day’s schedule
· Managing provider schedules when a provider is out sick or for PTO reschedules
· Print medical records for new patients for the next day appointments
· Check voicemails, return patient calls or send a flag to the correct department notating and detailing the information in the voicemail.
· Document patient voicemails by creating a flag or phone note
· Maintain office schedules by calling on appointments that are in red due to a patient cancellation or request to reschedule. Fill appointments by calling on waitlists
· Work as a team to make sure daily tasks are taken care of.
Maintains confidentiality of records.
Performs related duties as required.
Qualifications:
· High school graduate or equivalent preferred.
Friendly demeanor.
Some Computer knowledge/typing.
Good verbal and written communication.
Knowledge, Skills and Abilities:
· Able to maintain confidentiality.
Able to be friendly, helpful and courteous to patient’s in-office and on the phone.
Ability to understand and perform duties of position
Able to complete a complex variety of tasks in an organized manner.
Thorough knowledge of policies and procedures relating to position.
Ability to look ahead and monitor the appointment schedule to ensure appointments are scheduled strategically
Able to treat patients and staff with consideration and respect.
Able to multitask and work under pressure.

Starting wage is $18-24/hour depending on experience.

Responsibilities to Include but Not Limited to:
· Provide excellent care to our patients in a professional and courteous manner
· Assist providers with patient procedures
· Administer injectable medications
· Perform phlebotomy
· Triage patient calls
· Clean and stock exam rooms daily
· Send in patient prescriptions to their designated pharmacy
· Administers infusible, IV medications
· Other duties as required
· Multiple offices – travel is a possibility
Qualifications:
· Medical assistant or experience in a clinical office setting (preferred)
· Ability to work effectively as part of a team and to collaborate with team members from other departments
· Ability to demonstrate highly effective interpersonal communication skills

· Strong computer skills (medical record software experience a plus)
